There are two ways to make an appointment:

  • Call the EAP between 8am and 5pm Hawaii Standard Time.
  • Begin an online referral request by accepting the terms and conditions below:
  1. Please note that we do not monitor emails generated from this page 24/7. By using this online request for services, you agree that you are currently at no imminent risk of harming yourself or others.
  2. If you are needing immediate support, or at imminent risk or having thoughts of harming yourself or others, please immediately call the EAP at the number listed above or call 911 for immediate support.
  3. You must have one working phone line available with an activated voicemail box where voicemail messages can be received. The EAP will assume that a detailed voice mail message can be left on the phone number you provide on the online form. If you would not like a voicemail message left, please inform us in the Questions/Comments section of the online form.
  4. You should expect to receive a phone call directly from the EAP within 24 - 48 business hours (excluding holidays and weekends) to schedule your appointment or gather additional information so we can pass you to an affiliate provider who will contact you directly.
  5. You understand that the goal of the EAP is to offer an appointment time within two weeks of this request. However, if your only availability is outside normal business hours (M-F 8am-5pm), please be aware those appointments can take significantly longer to schedule.
  6. Please be aware that the EAP is not affiliated with any insurance company and cannot guarantee to connect you with an EAP provider that also accepts your health insurance for ongoing care.
  7. Please be aware that late cancelations or no-shows to your EAP appointment count against your total allowable EAP sessions.
  8. If you feel that the EAP provider is not a good fit after the first session, call the EAP before scheduling a second session to receive a re-referral. No re-referral is available if a second session has been created.
