Making a Drug/Alcohol Referral







How to refer an Employee to EAP for Drug or Alcohol Policy Violations:
It's always easier when employees call the EAP for confidential support for their use of alcohol or drugs BEFORE a violation happens.
But when employees violate DOT regulations for safety-sensitive employees, or when employees violate company drug/alcohol policies, they can be referred to the EAP.
For DOT violations (FMCSA, FAA, FTA, FRA, Pipelines, USCG) please complete the SAP Referral form and have the employee contact us. When the employee calls, we will make an assessment appointment, evaluate the need for any treatment/education, re-assess the employee once this is complete, and the SAP will send an initial letter as well as a letter of completion (with a testing schedule for up to five years).
For Non-DOT violations, please complete the SAP Referral form as well as the release of information on the second page. Send this to us and have the employee contact us. We will then schedule a similar process and let you know when the employee has completed our process and is ready to be tested for return to work.

How to make a Drug/Alcohol Referral:
For most industries (especially for safety-sensitive employees) the first priority is to make sure the employee is no longer performing safety-sensitive duties.
Call for a supervisory consultation with EAP to discuss ways to handle the situation and to decide whether a DOT-SAP or Non-DOT referral is appropriate. These calls are free, unlimited, confidential.
Download and complete the SAP Referral Form.
When informing the employee of the SAP referral, express your concern and a desire to help the employee resolve whatever led up to the violation or positive test. Stress the employer's focus on safety.
If this is a Non-DOT case, have the employee sign the referral form, giving his/her permission to have EAP disclose only specific, limited information to the employer.
Encourage the employee to call the EAP as soon as possible and their job status until they complete the SAP process and are ready to return for a follow-up test.

Follow up in a few days to see if they connected with the EAP.
EAP will inform you in a week of the employee does not follow through, or call the EAP any time to get the status of the case.
